Appointments & Fees:

My fees are in the standard range for licensed psychologists in the San Francisco Bay Area.  As my rates are occasionally adjusted and vary based upon the services requested, please call me for my current fee.  I do offer a sliding scale for those who demonstrate financial need.

Forms:

For new and current patients, I provide forms for: Informed consent; HIPAA Notice of Privacy Practices; and New Patient Intake, which can be accessed through my website. They require Adobe Acrobat Reader, if necessary, you can download it for free by clicking here.

This document explains my policies procedures, information about confidentiality, and includes an agreement for psychotherapy services. Please feel free to bring up any questions about this document when we meet.

I am required by law to provide you with a copy of the HIPAA Notice of Privacy Practices.

After reading the Informed Consent, Policies and HIPAA Notice of Privacy  please sign the Acknowledgement of form, which indicates that you have read and all forms.  Please bring this to our first session.

Prior to our first session, please complete my Client Intake Form.   This will help you share important details with me and speed up the process of my getting to know you.
